INTAKE FORMS
To the left are links to my intake paperwork if you wish to download, print, and fill out the forms to bring to the first session. Otherwise, I ask people to arrive 15 minutes early to complete the paperwork. I'll leave the paperwork on a clipboard on the countertop in my waiting room, if I"m in session when you arrive. If we meet via telehealth, you can download, print, and mail the forms or I'll USPS mail you the forms. RECORD REQUEST
If requested, I am happy to provide you a copy of your behavioral health records.
COMPLAINTS
If you need to file a complaint in Texas, you may call the Health Professions Council toll-free complaint referral system: 1-800-821-3205. This automated, statewide number routes a complainant to the appropriate licensing agency. You can also submit a complaint using the Texas Behavioral Health Executive Council's (BHEC) Council’s complaint form. The completed form can be sent by email to Enforcement@bhec.texas.gov or by mail to 1801 Congress Avenue, Suite 7.300, Austin, Texas 78701.
