Intake Forms, Requesting Your Records, Filing a Complaint, and BHEC Contact Information

I have intake paperwork, which I will give you the first time we meet. If we meet via telehealth, I'll USPS mail the forms. 

 

If requested, I am happy to provide you a copy of your behavioral health records. 

 

If you need to file a complaint in Texas, you may call the Health Professions Council toll-free complaint referral system: 1-800-821-3205. This automated, statewide number routes a complainant to the appropriate licensing agency. You can also submit a complaint using the Texas Behavioral Health Executive Council's Council’s complaint form. The completed form can be sent by email to Enforcement@bhec.texas.gov or by mail to 1801 Congress Avenue, Suite 7.300, Austin, Texas 78701.
